National Healthcare Group Polyclinics (NHGP) forms the primary healthcare arm of the National Healthcare Group (NHG). Its nine polyclinics serve a significant proportion of the population in Singapore.

This annual report was titled “Transforming the Primary Care Ecosystem in Singapore”. We brought this to life by basing our design on a tangram. Arranged in the shape of a butterfly, it was used to signify the metamorphosis of the primary care ecosystem. The tangram is also a representation of how every piece of the structure supports each other, coming together to form a desired goal.
